Czy ktoś z Was używa jakiegoś modułu do budowy formularzy internetowych?

Z racji ceny jestem najbardziej przychylny do ostatniego - WebForms Pro 2. Z tym że chciałbym poznać najpierw Wasze opinie. Czy ktoś z Was używał już go, lub innych?
Jak rozumieć licencję (mój angielski jest bardzo kiepski):
Domain License - extension can be activated on single store domain. Example: if your store has 2 domain names store1.com and store2.com, extension can be activated only on store1.com with serial number provided.
Server License - extension can be activated on multiple domains linked to one IP address or within Magento installation (if your store has multiple IPs). Example: if your store has lots of domains, extension will be activated on all of them. ]

Czy po wybraniu "Domain License" będę mógł używać tego modułu na sklepie który mam na subdomenach http://www.sklep1.domena.pl, http://www.sklep2.domena.pl, http://www.sklep3.domena.pl czy tylko na http://www.domena.pl.
Proszę o odpowiedź, a może znacie inne lepsze moduły bądź sposoby do budowy formularzy?
